Levofloxacin is a third-generation fluoroquinolone antibiotic that kills bacteria by disrupting their DNA replication machinery. It is available in oral tablet, oral solution, and intravenous formulations, as well as ophthalmic (eye drop) solutions under the brand names Quixin and Iquix. Because it is active against such a wide range of bacteria — including the organisms that most commonly cause respiratory tract infections — it has earned the designation of a "respiratory fluoroquinolone," a label it shares with moxifloxacin.

What Is Levofloxacin Used For?

How Does Levofloxacin Work?
Levofloxacin belongs to the fluoroquinolone class of antibiotics and works by targeting two enzymes that bacteria cannot survive without: DNA gyrase (topoisomerase II) and topoisomerase IV. These enzymes manage the coiling, uncoiling, and repair of bacterial DNA — processes that must occur continuously during bacterial cell division, gene expression, and DNA repair. By binding to and inhibiting these enzymes, levofloxacin traps them in a state where they cause irreversible breaks in the bacterial DNA double strand.
The resulting double-strand DNA breaks are lethal to bacteria, making levofloxacin bactericidal — it actually kills bacteria rather than merely slowing their growth. Serious Side Effects

- Peripheral neuropathy (BLACK BOX WARNING): Nerve damage affecting the arms, hands, legs, and feet — causing pain, burning, tingling, numbness, or weakness — can occur rapidly and may be permanent. Discontinue immediately if symptoms develop.
- CNS effects: Seizures, confusion, psychosis, hallucinations, toxic psychosis, and increased intracranial pressure have been reported
- QT prolongation and cardiac arrhythmia: Levofloxacin prolongs the QT interval, which can trigger life-threatening irregular heart rhythms, especially in patients with cardiac disease or low potassium/magnesium
- Clostridioides difficile (C. diff) colitis: Can cause severe diarrhea and colitis, ranging from mild to life-threatening, during or after antibiotic treatment
- Aortic aneurysm and dissection: FDA has issued a safety warning about a potential link between fluoroquinolones and aortic aneurysm or dissection, particularly in elderly patients

Who Should Not Take Levofloxacin?
