Venlafaxine
Venlafaxine was the first serotonin-norepinephrine reuptake inhibitor (SNRI) approved in the United States, introduced in 1993. It is available in an immediate-release form (Effexor) and an extended-release formulation (Effexor XR), with the XR version being strongly preferred for once-daily dosing, improved tolerability, and reduced discontinuation risk. Warnings & Contraindications
BLACK BOX WARNING: Antidepressants increase the risk of suicidal thinking and behavior in children, adolescents, and young adults (under 25). Closely monitor for clinical worsening, agitation, and emergence of suicidal thoughts, especially early in treatment and after dose changes.
Contraindications: Concurrent or recent use of MAO inhibitors. Known hypersensitivity to venlafaxine or desvenlafaxine.
Blood pressure: Venlafaxine consistently raises blood pressure in a dose-dependent manner. Pre-existing hypertension should be controlled before initiating treatment. Blood pressure should be measured at each visit.
Discontinuation syndrome: Venlafaxine's short half-life makes discontinuation syndrome particularly common and severe. Even a missed dose may trigger symptoms. Tapering over weeks to months is recommended. In difficult cases, switching to fluoxetine (with its long half-life) and then tapering is a clinical strategy some prescribers use.
Narrow-angle glaucoma: Mydriasis has been reported with SNRIs; use with caution in patients with this condition.
