Warfarin is FDA-approved for the prevention and treatment of venous thromboembolism (deep vein thrombosis and pulmonary embolism), stroke prevention in atrial fibrillation, and thromboembolism prevention in patients with mechanical prosthetic heart valves. How It Works

By depleting functional vitamin K, warfarin prevents the activation of these clotting factors, slowing the formation of blood clots.

Because warfarin targets clotting factor production rather than existing circulating factors, its anticoagulant effect is delayed — full therapeutic effect requires 5–7 days as existing functional clotting factors are gradually cleared from the circulation. This is why rapid anticoagulation (such as for acute DVT or PE) requires initial bridging with a rapid-acting parenteral anticoagulant (heparin or LMWH) until warfarin reaches therapeutic levels.

Warnings & Contraindications

BLACK BOX WARNING: Warfarin can cause major or fatal bleeding. Regular monitoring of INR is required. Drugs, dietary changes, and other factors affect INR levels achieved with warfarin therapy. Instruct patients about prevention measures to minimize risk of bleeding and to report signs and symptoms of bleeding.

Contraindications: Active pathological bleeding (e.g., hemorrhagic stroke, active GI bleed). Pregnancy (especially first trimester and near term) — Category X; causes fetal warfarin syndrome and increases fetal hemorrhage risk. Recent or planned surgery involving the brain, eye, or spinal cord where bleeding would be catastrophic. Unsupervised patients with high risk of non-compliance.

INR monitoring: Warfarin's therapeutic window is narrow. INR must be checked regularly — often weekly or biweekly when starting or adjusting therapy, and at least monthly when stable. INR outside the therapeutic range — either too low (clot risk) or too high (bleeding risk) — is common and requires dose adjustment.
