Overview
Zoloft (sertraline) and Prozac (fluoxetine) are two of the most recognized antidepressants in the world. Both are SSRIs (selective serotonin reuptake inhibitors) introduced in the late 1980s and 1990s, and both helped establish a new era in psychiatry by offering treatments for depression that were better tolerated than the older tricyclic antidepressants and monoamine oxidase inhibitors. How Each Works
Both sertraline and fluoxetine block the serotonin transporter (SERT), preventing the reabsorption of serotonin from the synapse back into the presynaptic neuron. By keeping serotonin available longer in the synaptic cleft, they gradually enhance serotonergic neurotransmission in circuits involved in mood regulation, anxiety, and emotional processing. This is their primary mechanism, and it is the basis for their antidepressant and anxiolytic effects.

The Half-Life Difference

Fluoxetine itself has a half-life of approximately 1–4 days, and its active metabolite norfluoxetine has a half-life of 4–16 days. This means that after stopping Prozac, meaningful drug levels remain in the body for weeks. By comparison, sertraline's half-life of roughly 26 hours means it clears the body much more quickly.
This difference has practical clinical consequences. Patients who stop Prozac are far less likely to experience discontinuation syndrome (the dizziness, irritability, flu-like symptoms, and "brain zaps" commonly associated with abruptly stopping SSRIs), because the drug essentially tapers itself as blood levels slowly decline. For patients who have difficulty tapering off SSRIs or who have a history of discontinuation syndrome with other medications, Prozac's pharmacokinetics may be advantageous.
The flip side is that Prozac's long half-life also means it takes longer to clear the body if a problematic reaction occurs, and potential drug interactions can persist for weeks after the drug is stopped — a consideration when transitioning to other psychiatric medications, particularly MAOIs, which require a significant washout period after fluoxetine.
Approved Indications
Both drugs share FDA approvals for major depressive disorder, OCD, and panic disorder. Sertraline additionally holds approvals for PTSD, social anxiety disorder, and premenstrual dysphoric disorder (PMDD). Fluoxetine is uniquely approved for bulimia nervosa and (in combination with olanzapine) bipolar depression. Side Effects
